Mandatory Requirements Check
Financial Certificate of Incorporation with Corporate Affairs Commission (CAC) – photocopy
Evidence of Tax Clearance Certificate for the last 3 years (2013, 2014 and 2015) corresponding to declared turnover .. (photocopy)
Evidence of VAT Registration Certificate with TIN Number – (photocopy)
Company Audited Account for the last 3 years (2013, 2014 and 2015) duly signed and stamped with a professional Accounting/Audit Firm showing annual turnover (photocopies)
Insurances Evidence of Certification/Exemption from PRNCOM in accordance with the Pension Reform Act, 2004, including remittances of employee’s pension contributions/deductions.
Evidence of remittance of Industrial Training Fund (ITF) contributions in accordance with ITF Reform Act 2011
Evidence of compliance with Nigeria Social Insurance Trust Fund (NSITF).
Regulatory Submission of Interim Registration Report (IRR) as evidence of registration on the National Data Base of Contractors/Consultants/Service providers in line with Section 5(b) of the Public Procurement Act 2007 (BPP Registration).
A sworn Affidavit that none of the Directors has been convicted in any court of law for any criminal/financial offence.
A sworn affidavit confirming that all information presented and attached are true and correct of the company and that no officer of the Contracting Ministry, Department or Agency is a former or present director, shareholder ot has any pecuniary interest in the bidder

Additional Requirements Check
Corporate Comprehensive Company Profile including names of key personnel to be involved in the project, stating their technical qualifications and experience on similar jobs
Financial Evidence of Financial Capability or specific statement of financial support from a licensed commercial bank (original)
Past Experience List of similar jobs successfully executed in the last 3 years including letters of award of contracts, job completion certificates (please provide contact addresses of the clients for verification purposes  (photo copies
Regulatory Compliance Evidence of registration with relevant regulatory bodies such as (COREN, CORBON and etc) for Building and Engineering Projects
Evidence of current registration with the Pharmacists’ Council of Nigeria and NAFDAC for the supply of drugs and surgical consumables.

 

Evidence of WHO pre – qualification and PQS number for Supply of relevant medical devices like auto – disable Syringes
