ADDENDUM
RE: INVITATION FOR PRE-QUALIFICATION
1. Further to our earlier advertisement on the submission of Technical Bids for Routine Procurement in the Centre published in the Federal Tenders Journal, Blue Print and The Nation Newspaper of Monday, 28th November, 2016, some omissions have been observed under the “Requirements”
2. Consequently, interested contractors are to submit the following in addition to the requirements in the advertisement:
I. Sworn Affidavit Indicating that no officer of the Centre, or the Bureau of Public Procurement (BPP) is a former or present Director, Shareholder or has any pecuniary interest in the company as stipulated in Section 16(6)(f) of the Public Procurement Act (PPA)
II. Letter of Authorization from original manufacturers in accordance with approved policy for procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria, for Bidders of LOT II
III. Evidence that demonstrate having resources and experience sufficient to meet the qualifying criteria for each LOT
3. It should also be noted that:
• LOT I – stands as General Goods and Services (details and nature will be on the bidding documents)
• LOT IV – as Works (details and nature will be on the bidding documents)
4. In order to allow enough time for submission of bids, the closing date for the submission of bids has been extended to 23rd January, 2017 at 12:00 Noon

INVITATION FOR SUBMISSION OF TECHNICAL BIDS
In compliance with the section 25(2)(II) of the Public Procurement Act, the Federal Medical Centre, Azare, Bauchi State hereby invites interested, competent and reputable general contractors/suppliers, original equipment manufacturers and their authorized agents, pharmaceutical companies and their agents with relevant experience to submit their technical bids for Routine Procurement in the Centre under the National Bidding Category
1. ELIGIBILITY
i. Evidence of registration with Corporate Affairs Commission (CAC) by inclusion of Certificate of Incorporation
ii. Evidence of Tax Clearance for the last three years i.e. 2013, 2014, & 2015
iii. Evidence of current compliance with Pensions Reform Act, 2004 by inclusion of compliance certificate
iv. Evidence of current compliance with the Industrial Training Fund Amendment Act, 2011 by inclusion of ITF compliance certificate
v. Evidence of Registration with Bureau of Public Procurement (BPP) Database of Contractors by inclusion of the Interim Registration Report (IRR) with BPP
vi. Detailed Company Profile to include Memorandum and Article of Association
vii. Evidence of similar jobs experience including Awards and Job Completion Certificate
viii. Current License/Certificate and Registration with relevant Professional Bodies and regulators (in the case of Pharmaceutical companies)
2. SCOPE
LOT I – General Goods and Supplies, Works and Services
LOT II – General Hospital Consumables and Medical Equipment
LOT III – Drugs and Related Pharmaceutical Products
Note that the scope of works and supplies above does not constitute conclusive description of the items and bidders can only bid for one LOT
3. SUBMISSION OF BIDS
The Technical documents should be enveloped and sealed, stating the company name, and LOT on the reverse side of the envelop and addressed to:

4. CLOSING/OPENING OF TECHNICAL DOCUMENTS
All bidding documents must be submitted on or before 12 Noon on Monday, 9th January, 2017, while the opening of bids takes place immediately after closing time. Any bid submitted after closing time stands rejected
5. OTHER INFORMATION
Only suppliers whose technical bids fulfill the minimum eligibility requirements would be considered in subsequent Centre’s Routine Procurements under National Competitive Bidding
